Enhanced Care Management

Enhanced Care Management (ECM) is a Medi-Cal managed care benefit designed to improve outcomes for the most vulnerable populations. Through ECM, qualified Medi-Cal members receive no-cost, comprehensive care management services delivered by contracted providers like CCAC. 

Each managed care plan member is assigned a CCAC Lead Care Manager who coordinates across medical, behavioral health, housing, and social service systems. Services are person-centered and whole-person focused, ensuring members receive the right care at the right time. ECM is available exclusively to Medi-Cal managed care health plan members (CCAC is currently contracted by Anthem and CalViva Health to provide ECM services to members in the counties listed below). 

CCAC’s ECM program is unique because it combines clinical care coordination with environmental health expertise. We specialize in supporting individuals with asthma and other chronic respiratory conditions, addressing not only medical needs but also environmental and social factors that contribute to poor health. This integrated approach strengthens collaboration between health plans, providers, and community organizations, while reducing lapses of care through consistent follow-up and cross-sector partnerships. 

CCAC’s ECM program goal is to see improved outcomes for members, including fewer emergency department visits, stronger medication adherence, and better daily functioning. Together with our partners, CCAC is building a healthier Central Valley—one member, one family, and one community at a time.
